At Federal Comp and Wellness, our patient’s needs are our first priority.  Dr. Martens provides the best medical care possible to get you well and back to a rewarding life. Our case managers will help you step-by-step through the process of filing required paperwork for your workers compensation claims, advise you on important deadlines, and ensure that Dr. Martens has the most up-to-date information regarding your claim.  Dr. Martens has over 14 years of experience in workers compensation in his 18 year career as a physician.  Our entire staff understands that having an injury or occupational disease can impact your life and you will be treated with respect and compassion.   We offer Return-To-Work Programs and have an on-site Physical Rehabilitation center with experienced rehab staff.  We are here to help and we will get your claim open so that you receive the medical treatment you deserve.

DR. THOMAS E. MARTENS, D.O., M.B.A.

PHYSICIAN/OWNER

Dr. Thomas E. Martens, D.O., M.B.A.

received dual Bachelor’s degrees in both Microbiology and Chemistry at Arizona State University. He completed medical school at Des Moines University in 1998. In 1999 he finished his internship at Mt. Clemens General through Michigan State University. He trained in Family Practice at the University of Minnesota/Mayo Health System graduating in 2001.

Dr. Martens practiced in Phoenix, AZ until moving to Texas at the end of 2003. Once in Texas he focused his practice in occupational health. Dr. Martens has extensive experience in occupational health medicine, having treated both state and federal work-related injuries since 2003.

In 2014 Dr. Martens returned to academia to pursue and complete an MBA degree in health care administration at the University of Tennessee, Knoxville. Understanding both the business and clinical aspects of health care allows Dr. Martens a unique perspective to both care for his patients and operate a busy and expanding practice.

Dr. Martens enjoys practicing medicine and is uniquely able to bond with his patients to positively affect their health outcomes. His patients and colleagues realize this and, as such, his patient referral base is largely “word of mouth”, referrals from patient’s families and friends, professional colleagues, and health insurance companies. He and his dedicated staff strive to offer the highest quality of health care possible.

DAVID A. ZOLEZZI, MS, PA-C

PHYSICIAN ASSISTANT

David A. Zolezzi, MS, PA-C

is a nationally board-certified Physician Assistant specializing in occupational medicine and family medicine. He received his Bachelor’s of Science degree in Biology with a minor in Spanish at Seton Hall University. He later received his Master’s of Science degree in Physician Assistant Studies at Seton Hall University in 2017.

David is committed to protecting, promoting, and maintaining his patient’s health and well-being. He believes in giving every patient a choice in their care because he understands that no one knows their body better than themselves. As a provider at Federal Comp & Wellness, David prides himself in connecting with his patients on a level that allows for the most optimal care.

In his free time, David enjoys exploring all that Austin offers. He also enjoys indoor cycling and anything that involves his dog, Ruby. He is originally from Chicago, Illinois and in his spare time likes to travel back home to spend time with his family and friends.

FREQUENTLY ASKED QUESTIONS

Q: I was injured on my job, what do I do?
A: Report your injury to your supervisor immediately.

Q: Can I see any doctor of my choice?
A: Yes. As long as they accept OWCP.

Q: Which forms do I need to file a claim?
A: The CA-1 and CA-2 forms allow injured workers to file a notice of claim. The CA-7 form allows injured workers to claim wage compensation benefits.

Q: My employer will not report my injury. What can I do?
A: Escalate the issue to higher management.

Q: Can I choose my treating doctor?
A: Yes. As long as they accept OWCP.

Q: Is there a timeline for filing a claim?
A: In most cases, the CA-1 must be submitted by the agency to the Office of Workers’ Compensation Programs (OWCP) within 10 days of notification by the employee. The Federal Employees’ Compensation Act provides that a claim for compensation must be filed within 3 years of the date of injury. For a traumatic injury, the statutory time limitation begins to run from the date of the injury. For an occupation disease or illness, it begins to run when an injured employee with a compensable disability becomes aware, or reasonably should have been aware, of a possible relationship between the medical condition and the employment.

Q: Are workers’ compensation benefits taxable?
A: No.

Q: While unable to work and collecting temporary workers’ compensation disability benefits, my employer discontinued my health care coverage. Is this discontinuation of coverage permissible under the law?
A: Yes. But DOL will handle the deductions when notified.

Q: How do I request a change of physician?
A: To request a change of physician, put the request in writing, detail the reason why you wish to change physicians, include the new physician’s name, specialty, and contact information, and sign the request. This may be mailed to the U.S. Department of Labor, DFEC Central Mailroom, P.O. Box 8300, London, KY, 40742-8300.
